Parent Volunteer Agreement

If you are interested in volunteering in the classroom, on field trips, and/or at any other formal school function or activity (e.g., chaperoning a sporting trip) please complete the application and return it to the front office.

**Fingerprinting is required. The school's HR department will provide you with the proper paperwork once your application is approved.**

Vaccination is not required.
